Developing a “Culture of Safety” A year ago we wrote about the complex process involved in identifying, categorizing, and assessing medical error in health care in the United States (”Assessing Medical Error in Health Care,” Health Progress, November– December 2001, pp. 14-17). That article discussed the 1999 Institute of Medicine (IOM) report on medical error, “To Err Is Human,” and the erosion of public trust in U.S. health care.

As I was completing my residency at Childrens National Medical Center, I, as many of my third-year colleagues, found myself wondering, where to go from here? Everything in our previous experience had been laid out in precise order, like check boxes waiting to be completed. Completing residency was the last check box on the list. We now had choices–fellowship, primary care, advocacy, etc. Enter Information Technology (IT). I can honestly say that, if I had been seriously considering a clinical job offer, I would probably be working in a clinic somewhere today. The lack of job, however, opened me to trying something new and different. When I heard about the informatics position, in which I would be able to help create a new computerized order entry system at Childrens, my hospital, I listened. and accepted the offer. Flash forward to Day One. My first day working in IT was definitely an experience to be remembered. I did not have to arrive at work until 8 am! I was able to sleep in! I actually had an office (cubicle) with my own phone and a laptop! As a resident, we had a lounge/classroom and had to vie for phone and computer usage wherever we went. In IT I had my own phone number and could give my new extension to whomever and could receive direct calls and messages.

Dental implants are artificial substitutes for natural tooth roots. The implants-small anchors shaped like screws, cylinders or blades-are placed into the jawbone. Replacement teeth are attached to the part of the implant that projects through the gums.
Endosseous dental implants are becoming a common treatment option in the practice of dentistry and have shown great promise for being able to restore dental function and esthetics. Spontaneous exposure of endosseous dental implants is a complication that can be indicative of later implant failure, but few definitive studies have been conducted concerning this histological phenomenon. In this study, literature review of relevant clinical and laboratory studies, along with clinical observations, were conducted to determine the histopathological factors associated with the spontaneous exposure, and subsequent failure, of endosseous dental implant systems during phase I (implant phase) of treatment. Fifteen peer-reviewed studies (1997-2001) of endosseous dental implant complications and failure, published over a five-year period, were reviewed. Clinical observations (N=95) of maxillary and mandibular (edentulous and partial dentition) surgical placement of single-unit, endosseous dental implants were conducted over a three-month period, to gain further knowledge of treatment procedures. All patients were in good health, and no control group was used. The studies were initially categorized by ascending chronology to evaluate the development of a definitive etiology. The studies were then classified as comprehensive or specialized, and evaluated. Initial results indicated that several factors involving the technique-sensitive surgical procedure contribute to the primary, soft tissue, inflammation response of spontaneous dental implant exposure. It was also noted that no definitive, uniform protocol exists for diagnosing this complication. T. The construction of a new maintenance center will accommodate GOLs recent fleet expansion, reduce costs and centralize the Companys aircraft maintenance operations, said Constantino Junior, GOLs President. Currently, GOLs fleet is composed of 27 aircraft. In May 2004, GOL signed an agreement with Boeing, announcing the acquisition of up to 43 737-800 Next Generation aircraft, of which 21 aircraft are already firm orders for aircraft that will be delivered between 2006 and 2009, and 22 of which are purchase options. The construction of GOLs Maintenance Center will generate 250 jobs, attract new businesses to the region and increase the traffic to the airport. In addition, Tancredo Neves International Airport is one of the best airports in Brazil, with a superior runway, modern navigation instruments, and is primely located in a region with favorable weather conditions. The Missouri Supreme Court upheld the rights of municipalities to collect subdivision and maintenance bonds from developers in a case that pitted the city of Wildwood against the Home Builders Association of Greater St. Louis. The General Assembly amended Section 89.410 in 1999 by enacting Senate Bill 20. As amended, Section 89.410.2 allows the city council to accept a bond “in an amount and with surety and other reasonable conditions, providing for and securing the actual construction and installation of the improvements.” The pre-amendment statute referenced “conditions satisfactory to it” rather than “other reasonable conditions.” In addition Section 89.410.3 requires cities to release any escrow amount within 30 days of completion of each category of improvement or utility, although cities may retain 5 percent to release when all improvements and utility work has been completed. According to Vogel, the Home Builders Association of Greater St. Louis proposed the 1999 amendments, but when municipalities changed their ordinances to conform with the amended statute, the HBA sued. In the case at hand, the HBA brought a declaratory judgment action against Wildwood challenging the validity of the city ordinance that requires the posting of bonds for subdivision improvements.

When the previous statement on foreign exchange was discussed at the Accounting Practices Committee we were of the view that the statement should include a definition of the date of the transaction due to the confusion surrounding this concept. We consulted with our legal colleagues and were informed that, if an order was placed for goods or services, the date the order was accepted was the date of the transaction. We attempted to get this entrenched in the statement but failed at the APB level. After much confusion, it was generally agreed that the cost of the inventory in the above example would be R65O if no cover contract was entered into or if cover was taken but hedge accounting was not permitted. It was also generally agreed that if cover was taken and hedge accounting was permitted, the cost of the inventory would be R6oo (the spot rate at the date the deal was made) plus RS (being the cost of the premium attributable to the inventory, i.e. Ri5-Rio) or R6o5. Then along came AC 133 with its embedded derivative in a host supply contract concept. It stated that if the deal was done in a currency that was not the currency of one of the parties to the transaction and the product subject to the transaction was not routinely bought and sold in the currency in which the transaction was entered into, there was an embedded derivative in the contract that had to be separated from the host contract. In the above situation, the embedded derivative would have been a liability of R45 (R6,6o - R6,is). The only way that could understand this concept was to see the embedded derivative liability as an expression of sadness that the deal was entered into in dollars and not in Rand. If the Rand went the other way, it would have been an asset, i.e. an expression of happiness. (A banker once asked me jokingly if he could lend money to a company based on the value of its embedded derivatives in its supply contracts!)

Or maybe as I sleep I am rising in an elevator which stops between floors and just hangs there. Silence. Then maybe all the others on the crowded elevator start chattering a deafening cacophony. Maybe their clothes look like feathers as the elevator opens on nothing and they jump up, laughing, and seem to fly away in every direction as I look down at my unchanged body, as I step out into nothing (I cant stop myself), as I step out into silence, into weightlessness and perfect blue.You remember a woman wrapped in white, sitting in a rocking chair beside a dark window. Lasers and light-based hair removal technology are at the forefront of providing long-lasting relief for patients with excessive hair, and now new research shows promise in treating people with skin and hair of all hues. Lasers work to successfully reduce hair growth and thickness by releasing pulses of light that penetrate the skin and are drawn to the pigmentation of hair follicles. These follicles selectively absorb the laser energy and are subsequently destroyed by it. At the same time, the surrounding skin tissue is left unaffected, and patients experience minimal side effects. However, one early limitation of hair removal lasers was that no single system could effectively remove hair of every skin type. Dark-skinned patients sometimes experienced skin discoloration, and patients with white, gray, or light hair could not benefit from this treatment because their hair follicles were unable to absorb the lasers energy. Today thats no longer the case, according to research published in the February 2004 edition of Dermatologic Surgery. Thirty-six dark haired adults with varying skin tones received three consecutive treatments with a long-pulsed 1064 nm Nd: YAG laser over 4 to 6-week intervals. One month after the final treatment, the patients experienced successful hair reduction ranging from 58% to 62% on the face, and 66% to 69% on other parts of the body. Success was still noticeable 6 months after the final treatment, with a mean hair reduction of 41% to 46% on the face and 48% to 53% on the body.
ANAHEIM, CALIF. — Some individuals having a laser procedure for hair removal are getting the opposite of what they desire: new terminal hair growth adjacent to treated areas. This is real, and its out there, Dr. Ranella J. Hirsch said at the annual meeting of the American Society for Laser Medicine and Surgery. There seems to be a predominance in the chin and sideburn area. That appears to be only partially true, however, said Dr. Hirsch of the Wellman Laboratories of Photomedicine at Massachusetts General Hospital, Boston.

TelephoneA working electrical voice-transmission system was first demonstrated by Johann Philipp Reis in 1863. His machine consisted of a vibrating membrane that opened or closed an electric circuit. While Reis only used his machine to demonstrate the nature of sound, other inventors tried to find more practical applications of this technology. They were found by Alexander Graham Bell in 1876 when he was awarded a patent for the first operational telephone. This invention proved to revolutionize the way people communicate throughout the world.Bells interest in telephony was primarily derived from his background in vocal physiology and his speech instruction to the deaf. His breakthrough experiment occurred on June 2, 1875. He and his assistant, Thomas Watson, were working on a harmonic telegraph. When a reed stuck on Watsons transmitter an intermittent current was converted to a continuous current. Bell was able to hear the sound on his receiver confirming his belief that sound could be transmitted and reconverted through an electric wire by using a continuous electric current.
The telephone is a device for conducting spoken conversations across any distance beyond the range of the unaided human ear or the unamplified human voice. It works by transferring the atmospheric vibrations of human speech into a solid body and by converting those vibrations into electrical impulses sent through a conducting medium–originally metal wires but now optical fibers and electromagnetic microwaves as well. The word is a compound of two Greek words “tele” (”far”) and “phone” (”sound”) and the instrument is the most widely-used of all telecommunications appliances with hundreds of millions of telephones in use all over the world. On any given business day approximately two billion calls are placed just in the United States. The telephone is also the archetypal electronic “medium” in the sense of the word intended by Marshall McLuhan–an “extension of man”–but its social impact is grossly understudied in favor of the more readily observable “bully blow” of the television. Telephones are small and unobtrusive and their impact on our visible environment (except for the poles and wires) has not transformed our relationship to it.

AN ITALIAN who claims to be a qualified criminal lawyer was asked yesterday to explain himself to three Court of Appeal judges. Giovanni di Stefano, who has represented some of Britains most notorious criminals, including Kenneth Noye, Jeremy Bamber and Harold Shipman, was questioned by the court over his right to practise law in Britain. Mr Lynch replied: “Mr Di Stefano. I dont believe he is a representative of Paul Martin.” But the judge said that Mr Di Stefano, who said he worked for Italian law firm Studio Legale Internazionale, had signed written legal advice in connection with the appeal.

Despite an industry upheaval and two gravely ill sons, this health-care entrepreneur discovered the prescription for perseverance. Sadly, looming business woes were the least of the Langdons worries that year. In the summer of 1996, at age three, their son Spencer–twin brother of Tyler–was diagnosed with leukemia, warranting three years of chemotherapy and approximately $1,000 worth of medication each month. Faced with circumstances that would obliterate the future goals of most, the Langdons didnt fold, but instead expanded ATPs services to override the Medicare massacre. Making ATP a onestop shop by adding outpatient rehab to its services was the ultimate goal, but first the Langdons had to move the company out of their Carrboro, North Carolina, home (situated in a bedroom, no less) and into a Hillsborough, North Carolina, office complex. To Ted, outpatient was ATPs salvation. I really started seeing the effects [of the Balanced Budget Act] in 1997, says Ted, 33. Medicare-based agencies were laying off staff, and the first people to go were with contract companies. If they didnt go Out of business or merge with other corporations, agencies drastically reduced their pay rates to contract companies, lowering the therapists wages. Some just didnt pay at all–like House Calls, a statewide home-health agency in North Carolina whose owner was nabbed for allegedly double-billing his way to Medicare/Medicaid fraud in 1997. I tried everything I could to retrieve [the $14,000 House Calls owed], but they were sued by everyone, says Ted. They had everything frozen, and I wa s so far down on the lawsuit list that, basically, I never got paid.

The use of general anaesthesia outside hospitals is to be ended in Scotland after a 10 year old boy died in a dental surgery during a procedure to have a tooth removed. An inquiry into the death of Darren Denholm has severely criticised the doctor and two dentists involved and has recommended that they be reported to their professional organisations for disciplinary action. He recommended that action be taken to end the use of general anaesthesia outside hospitals equipped with resuscitation facilities. The Scottish health minister, Susan Deacon, responded immediately by promising to introduce such a ban. I share the sheriffs concerns, and that is why I have asked my officials to work with clinicians to phase out the use of general anaesthetic in these circumstances as soon as possible, she said. The inquiry report also criticised the decision taken by the General Medical Council in 1997 to grant Dr Evans-Appiah full registration despite a history of problems related to his competence. Doctors at Borders General Hospital raised concerns with the GMC in the early 1990s after becoming concerned about the standard of his work. He was later sacked by Falkirk Royal Infirmary in 1993 after he administered the wrong drug to a woman after a caesarean section. Experts at the fatal accident inquiry described his anaesthetic practices as dangerous, bizarre, inappropriate, and inept.
AN INQUEST jury has returned a death by misadventure verdict on a five-year-old boy who died after having a heart attack in the dentists chair. Bradley Legge suffered massive brain damage after the attack last year in a Portsmouth dental surgery, where he had gone to have two milk teeth removed. Spurred by the success of the first volume in its Encyclopaedia of Sports Medicine, the International Olympic Committee has initiated a second series of publications to address specific sports activities. This series is aimed at coaches, team doctors, athletic trainers, physical therapists, and other health-related professionals with academic preparation in the basic sciences. This book, written by three American authorities on the application of science and sports medicine to swimming, is the first in the series.The first two sections of the book provide an excellent introduction to basic concepts in physiology and biomechanics of importance in swimming. They are logically organized, skillfully presented, and for the most part, well-illustrated. The presentation of complex scientific information in a form that is not only acceptable but attractive to practitioners is fraught with an inevitable tension between simplicity and accuracy. Each attempt to describe a complex process in simple terms is accompanied by a shedding of qualifiers that, if carried too far, threatens the accuracy of the information being presented. The authors have been uncommonly skillful in meeting the challenge inherent in this aspect of their task. A further challenge in presenting such material is to maintain an appropriate balance between theory (or science) and practice. The authors have managed to do this in admirable style, with the practical significance of the theory illustrated convincingly.The remainder of the second section is devoted to a discussion of the techniques used in the four competitive strokes. These are essentially an updated condensation of the corresponding sections of Maglischos popular text, Swimming Faster (Mayfield Publishing Co., 1982), replete with photo sequences, line drawings, and occasional discussions of related research. Although mostly descriptive, these chapters also contain helpful observations on contentious issues and useful insights gleaned from the authors experiences as a college coach. Unfortunately, the photo sequences are no match for the quality of the text. The photography is uneven, with some sequences sharp and contrasting and others out of focus, blurred, and lacking contrast. In addition, the sense of movement that a good photo sequence can convey when laid out appropriately–that is, running from left to right across adjoining pages–is lost when the photos are stacked vertically in a left-right, left-right fashion.

One of Brian McNamees lawyers said Sunday he believed the Justice Department will open a criminal investigation into Roger Clemens denials of doping. Meantime, the chairman of a congressional committee said comments attributed to one of the pitchers lawyers could be interpreted as trying to intimidate a federal law enforcement official. “I think there will be a criminal prosecution after Wednesday, and that means there will be grand jury proceedings and subsequent proceedings,” said Richard Emery, one of McNamees attorneys. “I dont see theres any possibility that Brian has any jeopardy. I only see the possibility of Clemens getting investigated by Justice, whether or not Congress refers it.”
I used to love to go to parties. Sadly, that all changed once I became a criminal defense attorney. Below is the conversation that I have at every shindig that Ive attended over the last 10 or so years. (Invariably dollar signs flash in my new acquaintances eyes despite the fact that Im clearly wearing a suit unfit for a rack in a fancy law firm.)
Significant ethical risks prohibit criminal defense lawyers from arranging or posting bond for clients in all but the rarest of cases, but there may be tactical reasons for them to do so and circumstances where they may. But the committee also suggested that it would be rare to obtain genuine and voluntary informed consent from a client experiencing stress, anxiety and other pressures from confinement and the conditions of incarceration. The committee noted that civil rights lawyers historically posted bond for criminal defendants where it was likely that no one else would do so. It acknowledged that obtaining the release of a client from custody could be a practical or tactical goal in handling a criminal case, which could qualify the step as a legitimate cost of litigation. It also suggested there might be occasions in which a lawyer who is a friend of the clients family would reasonably expect the family to indemnify her for any loss.

Health Alliance Medical Plans last month announced its coverage of two over-the-counter drugs formerly available by prescription only: Wyeths Alavert and Procter Gambles Prilosec OTC. Health Alliance will require members who have prescription drug benefits to obtain a written prescription from their physicians for the new over-the-counter drugs in order to receive coverage. The news at Health Alliance follows Medco Health Solutions expansion of its Generics First program in June to include Wyeths analgesic Advil and allergy remedy Alavert.

Santiago Marquez joined the staff at First Eagle National Bank in Chicagos West Loop earlier this summer as a commercial lender. His goal is to translate the banks real estate expertise into something meaningful for the Hispanic market. Chapter 2, Acute Care, covers neurologic and orthopedic aspects of SCI and treatment approaches to minimize further injury and complications. Chapter 3, Rehabilitation, classifies impairments of sensation, motor function, micturition, defecation, sexual function, autonomic function, skin, and breathing. The management of these impairments, with the goal of minimizing disability, is outlined. Problems that might be faced in continuing care, such as posttraumatic syringomyelia, spasticity, and pain, are focused on in Chapter 4. The fifth and last chapter of this book, Participatory Planning, emphasizes involving the patient in goal setting and presents approaches for the planning process. The authors intended audience-physicians working with patients with SCI (particularly residents in neurology and rehabilitation medicine), physical therapists, occupational therapists, nurses, and others seeking a basic introduction to SCI-certainly will find this work useful. Other texts, including those listed in the references at the end of each chapter, would be better resources for more detailed information on the management of persons who have SCI. Physical therapy practitioners and other health care professionals would find the chapter on participatory planning particularly interesting and valuable.

More Utahns are paying their mortgages on time, and the state continues to boast one of the lowest foreclosure rates in the nation, according to a new report. For the three months ended March 31, Utahs foreclosure rate dropped to 0.57 percent, less than half the national rate and down from the states 0.87 percent rate in the first quarter of 2006, according to the Mortgage Bankers Associations delinquency survey. The last time Utahs foreclosures were lower was in the second quarter of 1998, when the rate was 0.56 percent.
Few Foreclosures: For those few San Diegans who can actually afford to buy a home in the county, the rate at which the homes are being foreclosed upon slowed dramatically last year. Marshall Prentice, the president of DataQuick, said that while theres always going to be a certain level of distress out there, right now, because of increasing home values, virtually everyone can sell or refinance if theyre really in trouble. While the trend in lower foreclosures was widespread across the state, the declines were strongest in Los Angeles and Ventura counties, which saw a 48.5 percent and 42.8 percent annual decline in the number of home foreclosures, respectively.

SAN FRANCISCO–(BW HealthWire)–Aug. 11, 1999– The American Academy of Cosmetic Dentistry recently announced that San Francisco based dentist, Dr. Jerry R. Bellen, has been awarded Accredited Member status in the Art and Science of Cosmetic Dentistry. Dr. Bellens accomplishment was obtained through the demonstration of exceptional skill and judgement before a panel of accredited dentists from the United States. The American Academy of Cosmetic Dentistry is the largest association of dentists specializing in Cosmetic Dentistry in the world.

(CCADS) has successfully completed its first graduating class of doctors.
Fifteen doctors from all over Northern California have completed a unique
ten month educational program designed to advance their clinical skills in
the area of cosmetic dentistry. Along with other recipients worldwide,
educational grants in the amount of $18,500 were awarded to each doctor in
this select group.

With thirteen programs scheduled through 2009, CCADS is the first of its
kind offering access to full scholarships worldwide to doctors and their
staff for comprehensive training. “Led by the top clinicians and
professionals in this field, our program is unsurpassed in the area of
secondary post-graduate education, helping to develop, manage and support
some of the most prominent programs in the world,” said Garrett Caldwell,
President of The California Center of Advanced Studies.

“We are very proud of this talented group of doctors. It is rewarding to
be able to personally train these doctors on a very special set of skills
that they can take back to their practice and provide the best care
possible. By awarding educational grants to qualified doctors, we are
raising the level of care provided. Im very excited to start the next
series of study clubs so that we can find the right doctors to train,” said

Recruitment scouts have been appointed to help raise the number of young people joining vocational training schemes. CITB-ConstructionSkills has appointed Alastair Melvin from Blyth, Brian Myers from Wallsend and Tracey Bowles, from Durham City to help recruit apprentices and encourage employers to take them on. The CITB-ConstructionSkills recent Skills Foresight Report shows that over 3,500 recruits will be needed each year in the region over the next five years to enable firms to meet current and future orders.

ConcreteConcrete is a hardened building material created by combining a chemically inert mineral aggregate (usually sand, gravel, or crushed stone), a binder (natural or synthetic cement), chemical additives, and water. Although people commonly use the word cement as a synonym for concrete, the terms in fact denote different substances: cement, which encompasses a wide variety of fine-ground powders that harden when mixed with water, represents only one of several components in modern concrete. As concrete dries, it acquires a stone-like consistency that renders it ideal for constructing roads, bridges, water supply and sewage systems, factories, airports, railroads, waterways, mass transit systems, and other structures that comprise a substantial portion of the U.S. wealth. According to the National Institute of Standards and Technology (NIST), building such facilities is in itself one of the nations largest industries and represents about 10 percent of the gross national product. Over $4 billion worth of hydraulic cement, a variety which hardens under water, is produced annually in the United States for use in $20 billion worth of concrete construction. The value of all cement-based structures in the United States is in the trillions of dollarsroughly commensurate with the anticipated cost of repairing those structures over the next twenty years.The words cement and concrete are both of Latin origin, reflecting the likelihood that the ancient Romans were the first to use the substances. Many examples of Roman concrete construction remain in the countries that encircle the Mediterranean, where Roman builders had access to numerous natural cement deposits. Natural cement consists mainly of lime, derived from limestone and often combined with volcanic ash. It formed the basis of most civil engineering until the eighteenth century, when the first synthetic cements were developed.The earliest manmade cement, called hydraulic lime, was developed in 1756, when an English engineer named John Smeaton needed a strong material to rebuild the Eddystone lighthouse off the coast of Devon. Although the Romans had used hydraulic cement, the formula was lost from the collapse of their empire in the fifth century A.D. until Smeaton reinvented it. During the early nineteenth century several other Englishmen contributed to the refinement of synthetic cement, most notably Joseph Aspdin and Isaac Charles Johnson. In 1824 Aspdin took out a patent on a synthetic blend of limestone and clay which he called Portland cement because it resembled limestone quarried on the English Isle of Portland. However, Aspdins product was not as strong as that produced in 1850 by Johnson, whose formula served as the basis of the Portland cement that is still widely used today. Concrete made with Portland cement is considered superior to that made with natural cement because it is stronger, more durable, and of more consistent quality. According to the American Society of Testing of Materials (ASTM), Portland cement is made by mixing calcareous (consisting mostly of calcium carbonate) material such as limestone with silica-, alumina-, and iron oxide-containing materials. These substances are then burned until they fuse together, and the resulting admixture, or clinker, is ground to form Portland cement.
